Project: Armsby Parkway Slay River Bridge Expansion

Meeting #1
4/8/11
Items:
1. Footing casements have begun. Concrete pours 4/17/11.
2. Steel arriving 5/7/11.
3. Roadway framing 5/21/11.
4. Project completion date fixed at 6/22/11. Refer to attached schedule.
5. Initiating Safety Meetings. Will report on Safety issues in these meetings.
6. New business:
a. Clearing of brush for new bridge location is 75% complete. Disagreement with city inspector regarding disposal of brush. Will price shredding brush and hauling away.
b. Permits improperly posted. Received warning from city inspector. Permits have now been prominently posted on the job trailer.
c. Dump trucks must be covered to avoid spilling debris on freeway.

Meeting #2
4/15/11
Items:
1. Footing casements framed, flange iron in place. Ready for pour on 4/17/11.
2. Steel still on target 5/7/11.
3. Revised schedule attached.
4. Safety: no lost man-hours. All subcontractors shall remind crews that jobsite is non-smoking. Workers shall wear hardhats.
5. New business:
a. Brush clearing complete. Brush shredded and shall be used as mulch for project groundcover.
b. City inspector objected to number of fire extinguishers; two were posted but three required. A third extinguisher has been procured.
c. Important issue: existing bridge has occupant under west span, Troll named 'Herman'. Informed Herman of project schedule and original bridge demolition on 6/15/11. Herman refuses to leave. Must revisit with city council.

Meeting #3
4/22/11
Items:
1. Footings sabotaged by Herman. Pour will be rescheduled for 4/26. No anticipated slippage of schedule.
2. Steel still arriving 5/7/11.
3. Revised schedule attached.
4. Safety: no lost man-hours. Workers shall wear hardhats at all time while on site, even during lunch. Subcontractors shall remind crews not to taunt Herman.
5. New business:
a. City inspector objected to mulch piles in lay-down area as they represent a fire hazard. A FIRE HAZARD. Mulch piles hauled away to landfill.
b. Cage for Herman procured. Goats for bait procured. Trapping of Herman scheduled for 4/25.
c. Perimeter fence blew down over the weekend. Will provide 8' supports and bracing.
d. Heavy rainfall may delay work.

Meeting #4
4/29/11
Items:
1. Herman trapped. Demands carrots. Carrots have been procured.
2. Steel arriving 5/8/11, one day late. No change to end schedule.
3. Revised schedule attached.
4. Safety: four lost man-hours. Casement worker walked too close to Herman's cage. Worker was treated at hospital and released. Signs and safety fencing posted.
5. New business:
a. City inspector tagged job for using unbonded company to perform troll trapping. Also cited lack of licensure for Large Humanoid Cage. Bonding documents have been submitted, license procured and fines paid.
b. Concrete poured in casements; cure testing tomorrow.
c. No delay owing to rain; however, drainage pumps procured.

Meeting #5
5/6/11
Items:
1. Herman will not stop howling. Noise complaint from neighborhood association. Procured new gravel for neighborhood play areas. More carrots for Herman.
2. Steel on target for 5/8/11. Rigger and tower crane on site 5/7/11.
3. Updated schedule attached.
4. Safety: no lost man-hours. Workers shall wear steel-toed boots as well as hardhats. Workers shall refrain from poking Herman with pieces of rebar.
5. New business:
a. Architect on site to develop new habitat for Herman under new bridge. Features will include 50% larger cave area, garden with carrot patch, electronic security system to alarm if goats attempt to cross bridge.
b. City Inspector wrote project up on noise ordinance violation. Fines paid.
c. City Inspector, not content yet, wrote project up on improper sanitation and pest control in care and keeping of Herman. Herman bathed with power-washer. Herman unhappy with project, city inspector, goats, unfairness of life.

Meeting #6
5/13/11
Items:
1. Deal made with Herman. Herman has stopped howling.
2. Steel arrived on schedule, uprights in progress.
3. Roadway framing target date pushed to 5/23/11.
4. Updated schedule attached.
5. Safety: no lost man-hours. Workers were advised not to be on site between the hours of 12pm and 2PM on 5/12/11.
6. New business:
a. City Inspector was due on site between 12PM and 2PM on 5/12/11. During that time, Herman somehow procured key to cage and escaped. No sign of City Inspector.
b. Herman has agreed to behave and will no longer be caged. Cage returned. Additional carrots procured.
c. Excavation of new Herman habitat begun. Herman helped.

Meeting #7
5/20/11
Items:
1. Uprights complete. Owing to rain, laying of spans is behind schedule. Anticipate spans complete 5/23.
2. Roadway framing cannot be complete until 5/27. No anticipated shift in project end-date.
3. Updated schedule attached.
4. Safety: no lost man-hours. Workers and Herman shall wear hardhats. Herman not required to wear steel-toed boots.
5. New business:
a. Herman habitat dug, walls framing in. Herman happy, stated nobody ever does nothing nice for Herman ever before.
b. No citations for project for past ten days. Refreshing.
c. Roadway framing crew on site 5/24.
d. Reminder from City: failure to meet 6/22/11 project end date will result in compounded liquidated damages. Weather slippages notwithstanding.
Meeting adjourned.

Meeting #8
5/27/11
Items:
1. Continuing heavy rains caused considerable project slowdown last week. Pumping underway.
2. All spans but one complete. Roadway framing has not begun. Project can avoid extending project end date if time can be made up in roadway pour.
3. Updated schedule attached.
4. Safety: no lost man-hours. Herman caught a rigger who slipped off the East 2 span and saved him from falling in the river.
5. New business:
a. Hero party for Herman. Carrot cake served. Herman very happy, cried.
b. New City Inspector visited jobsite. Herman escorted around site. No citations from Inspector.
c. Roadway framer on site today, will remain available to work as soon as remaining span can be rigged.

Meeting #9
6/3/11
Items:
1. Spans complete, roadway framed.
2. Unavoidable delay in gravel order. Materials will not be on site in time for 6/10 roadway pour. This will delay 6/15 bridge shutdown to following weekend, and project end date must slip to no earlier than 6/27.
3. Updated schedule attached.
4. Safety: no lost man-hours. Herman ate too many carrots, but elephant indigestion tablets procured from zoo. Herman very sick behind #3 piling but otherwise okay. Workers advised to avoid #3 piling for several days.
5. New business:
a. Meeting with city attorneys. Liquidated damages will be assessed owing to project end-date slippage.
b. Informed Herman that resulting budget cuts will require trimming some features of habitat, including picket fence and industrial-strength swing set. Herman unhappy, wishes to eat lawyers.
c. Underlayment will be provided next week in case gravel order accelerates.

Meeting #10
6/10/11
Items:
1. Miraculous delivery of large order of gravel! WE ARE POURING ROADWAY.
2. May be tentatively able to revert to 6/15/11 road shutdown schedule. 6/22 end date possible.
3. Updated schedule attached.
4. Safety: no lost man-hours. Workers shall wear hardhats, steel-toed boots and dust masks. Herman shall wear hardhat and pants.
5. New business:
a. Herman habitat interior complete. Herman is moving house.
b. Herman bribed with truckload of carrots to avoid making handprints in roadway pour.
c. Cure test 6/13, surfacing 6/14, striping morning of 6/15. It's going to be a close one.

Meeting #11
6/17/11
Items:
1. Roadway completed on schedule with two hours to spare. Bridge changeover complete; new bridge in use.
2. Demolition of old bridge underway. Herman very sad.
3. Revised project end date of 6/22 looking good. No liquidated damages.
4. Revised schedule attached.
5. Safety: no lost man-hours. Workers shall refrain from barbecuing under new bridge with Herman. Workers need no longer wear dust-masks.
6. New business:
a. Complaint from neighborhood association that all gravel, every rock and pebble, from neighborhood playground disappeared last week. Asked Herman if he knew anything about it. Herman claims he is just a simple troll. New order of gravel for playground placed, will arrive next month.
b. City Inspector commended project for excellent containment of trash and groundwater runoff. Herman gave him a big kiss.
c. Demolition haul-away scheduled 6/20/11, project site breakdown 6/22.
Meeting adjourned.

Meeting #12
6/24/11
Items:
1. Project complete. Old bridge gone, Herman's old cave infilled, new vegetation planted.
2. Herman's fence and swingset complete.
3. Jobsite cleaned and trailer removed. City signed off on 6/22 substantial completion.
4. Safety: no lost man-hours. However, Herman very sad to see us go. Let Herman keep hardhat.
5. New business:
a. Close-out party held. Herman ate all the gumbo, crawfish, carrots. Fortunately still had some elephant stomach acid tablets.
b. Old City inspector's hardhat found stuffed under trailer. Curious that.
c. Have final surprise for Herman. Tonight, 4AM, during minimal bridge traffic, goat will be released on new bridge span and will be urged to travel eastbound. Making arrangements for video recording.
